Functional Results After Limb Salvage Surgery For Osteosarcoma

Abstract
After the limb salvage surgery for malignant bone tumors, the functional results are variable according to reconstructive procedures. Among the various reconstructive methods for the limb salvage surgery, prosthetic reconstruction and resection arthrodesis are the most popular methods. In this study, the merits of these two procedures were compared by reviewing the functional results of groups of prosthetic reconstruction and resection arthrodesis caused by resection for osteosarcoma. Comparisons were based on the functional evaluation system which was proposed International Symposium On Limb Salvage (ISOLS) in 1993. Sixteen cases who had been treated with prosthetic reconstruction or the resection arthrodesis at the site of the distal femur or proximal tibia for osteosarcoma between April 1992 and December 1997 were reviewed. The 10 cases underwent prosthetic reconstruction and the 6 cases underwent resection arthrodesis. All patients underwent chemotherapy. The average follow-up period was 3 years and 9 months. At the final follow-up, survival status were continuous disease free in 11 cases, alive with disease in 1 cases, and died of disease in 4 cases. Postoperative complications included superficial infection in 4 cases, deep infection in 1 case and delayed union in 1 case. The average functional score of prosthetic reconstruction was 69%, and that of resection arthrodesis was 61%. The functional results of prosthetic reconstruction were superior but had not the statistical significance(P>0.05). The functional scores of the prosthetic reconstruction were superior in terms of the functional ability, emotional acceptance, use of supports and gaits, while the resection arthrodesis were superior in pain and walking ability.
